Log in using the mobile number of the insured person (IP) registered with ESIC. An OTP will be sent for verification.
Fill out the online application form completely.
Upload the required documents, including your passport-size photograph, insured person's passport-size photograph, birth certificate or matriculation certificate (proof of date of birth), NEET (UG) 2024 admit card, affidavits (if applicable), etc.
After approval, a downloadable "Ward of IP Certificate" (PDF format) will be available on the ESIC website.
